How do I schedule an appointment?


Before booking your first appointment at Elite One Athlete, we will hop on a free 20 minute discovery call to ensure that we are the best providers for you and your goals! We also walk you through services we offer so you have a better understanding of the quality of care you will receive and how we are different from other physical therapy clinics. If we recommend our services and you are ready to commit to changing your life, we will schedule you for your initial evaluation.



What are your prices?


Every patient’s care looks a bit different from one another, and services and pricing may reflect that. Please book a free discovery call if you are interested in our services and we can discuss pricing!



Are in networks with any insurance companies?


No, Elite One Athlete is out of network with all insurances.


We believe that an out of network approach is best for the patient because we will have no restrictions on what the best plan of care is for you. Your plan of care is completely determined by what we as the clinicians and you as the patient determine is the best course of action.


Many times, I will have patients who come to me looking for assistance with one joint such as their neck or shoulder, but another day they come in and have knee pain. Because we are a cash pay clinic, we can address the entire body and we can help you with both concerns! If we were in network, we would not be able to help you with your secondary concern without a new evaluation or plan of care.



Why are you a cash-based clinic?
You’re In Control.

At Elite One Athlete, we believe a cash-based model benefits our patients the most. We want you to not only feel better and get back to baseline, we want you to exceed and get back to doing what you love. By taking insurance, we would not be able to do that. Once your pain is low (not even 0/10) and your range of motion is “acceptable” (not even functional), insurance often will say you are better and they will no longer pay. We believe that this is something for the patient and the clinician to decide, not someone that has never met you.

Most Time Efficient.

Because you will be seen one-on-one with a doctor of physical therapy, your sessions will be more effective. There will be no waiting around for the PT to finish up an exercise or manual technique with another patient so you can get right on in to the therapy technique. Because of this, you will only need to be seen for physical therapy one, maybe two times a week. That means less time spent in therapy the day of, less time spent in traffic, and less time in the duration of your physical therapy program! After each session, you will be provided with a digital exercise plan of your home exercise program with daily reminders if you choose. You will feel better quicker with a smaller time commitment!

More Cost Effective.

Here’s the kicker! Opting for a cash-pay model, patients will often spend less money out of pocket! Here’s why…

Let’s say you go to an in network clinic. Because you only get to see the PT about 10-15 minutes of that session, you come in 3 times a week with the average copay of $75 ($225/week in co-pays). You stay on the plan of care for an average of 12 weeks ($2,700 in co-pays for the 3 months of therapy). You complete your therapy and receive a bill for an average of another $600 in fees the insurance company does not pay ($3,300). Meanwhile, none of this is covered because you have not met your ($5000?) deductible. This means you’re in therapy for 3 months, have paid an average $3,300 in bills that are not covered because your deductible has not been met, and you may or may not feel a little bit better.

Now let’s say you take the cash-pay approach. You buy a package of sessions upfront, none of which cost as much as the amount in co-pays and bills. You get quality one-on-one care with a doctor of physical therapy for typically one session a week. In that one session, you are already spending more time with your doctor of physical therapy than you would in 3 sessions at an in network clinic. Not only are you paying less, you are having more time with your physical therapist which results in fewer months in rehab and fewer trips in traffic to the clinic. On top of that, upon request we will provide you with a “superbill” with your appropriate codes that can be submitted to insurance for possible further reimbursement. If that isn’t enough, you can also pay using an HSA or FSA account using pre-tax dollars. Better quality of care, less time, less money.


What should I wear for my appointment?


We are going to get you moving so please wear comfortable, non-restrictive clothing and athletic footwear. If you wear a specific type of shoe for your sport, please bring them with you.



What do the initial appointments consisted of?


Initial appointments allow for a hands-on approach to the assessment of each individual, appropriate exercises, education on the findings, and a plan of care tailored to each individual’s needs. Your physical therapist will advise you on what is the best way to approach the problem, and will consult with you to see if you are in agreement with the course of action. Please note that all plans of care are entirely dependent on the individual’s presentation.


Do I need a physician’s referral for physical therapy?


No. Arizona allows direct, unrestricted access to physical therapy services, meaning you do not need a physician’s referral to seek physical therapy. If you do plan to submit your claim to insurance however, some insurances may require a physician referral for approval.


How do I pay?


We accept cash, checks, card, Apple Pay, FSA, and HSA We do ask for a credit card on file in case of our late cancel/no show policy so applicable fees can be charged.


What is an HSA and FSA account, and can I use them for my visits?


Yes! We accept both HSA and FSA accounts! Some insurance plans allow eligibility for a Health Savings Account (HSA) or a Flexible Spending Account (FSA). They help individuals save money and prepare for upcoming medical expenses. Both of these accounts allow for tax savings because you can add the money to your account “pre-tax.” For more information please consult with your employer or administrator.


Can you keep my primary care physician (PCP) or other doctor updated with my care?


Absolutely! If you would like Elite One Athlete to keep in contact with your PCP, surgeon, or another doctor, please provide us with their contact information (Including name, company, email, phone number, and fax) and written consent to release this information.
